The Case Manager will provide housing retention services through a person-centered, culturally responsive, housing first model with 90-100 low-income family and individual households. Services and support will be provided in both English and Spanish, as well as any additional languages needed utilizing interpretation services. Primary job duties will include voucher application support, accepting referrals from the Clackamas County Social Services Division Rent Relief Team, participating in Housing Authority and Housing & Community Services Division RLRA voucher trainings and orientations, and collaborating and problem solving with applicants, tenants, and county staff and services. The Case Manager will also help veterans access other needed support and services to include increasing income, address physical and behavioral health needs, access veterans benefits, and various other supports depending on individual needs. In this position, the Case Manager will provide housing placement and housing retention services using a person-centered, housing first model to support approximately 20 homeless or formerly homeless individual and family veteran households who meet U.S. Department of Housing and Urban Development's (HUD) chronic homelessness eligibility criteria. The Case Manager in this position will use a strengths-based, person-centered approach to provide supportive service for veterans and their families that are experiencing homelessness or at risk of homelessness.
