below and note the following:

Enter dates in the format: mm/dd/yy.

DO NOT use double quotes
( " " ) in the Business Name field. Single
quotes and HTML code are acceptable.

DO NOT include "http"
in the Web site field
(it's already included and will cause an error on the display form)..
