The Case Manager will also help veterans access other needed support and services to include increasing income, address physical and behavioral health needs, access veterans benefits, and various other supports depending on individual needs. In this position, the Case Manager will provide housing placement and housing retention services using a person-centered, housing first model to support approximately 20 homeless or formerly homeless individual and family veteran households who meet U.S. Department of Housing and Urban Development's (HUD) chronic homelessness eligibility criteria. The Case Manager in this position will use a strengths-based, person-centered approach to provide supportive services for veterans and their families that are experiencing homelessness or at risk of homelessness.
