This position will be providing case management services for adults with significant mental health and substance use issues. We are looking for an energetic, resourceful, warm and empathic individual who works well independently and as part of a team. The incumbent must have an excellent ability to interact with a diverse client base while maintaining professionalism in a stressful environment. The position will include in home visits and transporting individuals as part of a service delivery model.
